Medication
Occasionally it becomes
necessary for a child to take medication during
the school day. Only prescribed medications will
be given at school. In the event a child needs
medication during the school day, the following
procedures apply:
ADMINISTRATION OF
PRESCRIPTION MEDICATION WILL BE DONE ONLY:
· with written authorization from the doctor
· with written authorization from the parent or
guardian
· if medication is in a container labeled by the
physician or pharmacy
If a child will be taking prescription
medication regularly during the school year,
parents will need to sign a medication form,
available from the nurse.
If a child is taking short-term medication (less
than 10 days) parents may send a written note.
The note must include the name of the
medication, physician’s name, dosage amount,
time medication is to be given, and possible
side effects
MEDICATION MUST BE
BROUGHT TO THE NURSE. IT IS LOCKED IN A CABINET
AND IS DISTRIBUTED BY AUTHORIZED SCHOOL
PERSONNEL ONLY.
